How to Use This Section-by-Section Guide

Each section below corresponds to a section of the Appendix A disclosure form you'll complete. Click on any accordion to expand detailed help for that item.

Answer Options: Most items have 4 choices: Working, Not Working, Do Not Know if Working, or None / Not Included.

Seller Occupancy

This determines which disclosure form you should use

If you select "Seller is not occupying the subject property" AND have no knowledge of defects, you may be eligible to use Appendix B instead of Appendix A.

Water Systems

All systems related to water supply, heating, treatment, and drainage in your home.

HVAC & Climate Control

Heating, ventilation, and air conditioning systems that control your home's temperature and air quality.

Gas & Fuel Systems

Pools & Outdoor Features

Kitchen Appliances

Safety & Security Systems

Other Systems

IF YOU ANSWERED "Not Working" to any items, please explain.

This is a REQUIRED field if you marked anything as "Not Working". For each item:

  • State which item is not working
  • Describe the problem specifically
  • If known, state when it stopped working
  • If you have repair estimates, include the cost

Example: "Water heater - no hot water, unit is 18 years old and needs replacement. Estimated cost $1,200-1,500."

Property Conditions (Questions 1-49)

These Yes/No questions cover zoning, flood zones, water damage, structural issues, roofing, pest control, environmental hazards, legal issues, and more.

If you answered YES to any of the items, list the item number(s) and explain

This is a REQUIRED field if you answered "Yes" to any questions 1-49. For each "Yes" answer:

  • State the question number
  • Provide detailed explanation of the issue
  • Include dates, repairs made, costs, contractors used
  • Attach documentation if available

Example: "Q12 - Foundation repairs in 2018. Noticed cracks in south wall, hired ABC Foundation to install 6 piers, cost $8,500. No issues since. Invoice attached."

Are there additional pages attached?

If your explanations don't fit in the provided space, you can attach additional pages. Indicate "Yes" and specify how many pages you're attaching. Each additional page should be labeled with your property address and initialed.
